Finally, over the past three years, the generic oral contraceptive market has included us and only one other generic competitor. However, two additional competitors have entered during the second half of fiscal 2004. As a result, we anticipate some loss of market share and lower pricing on certain oral contraceptive products during fiscal 2005. Despite these challenges, we believe our generic oral contraceptive sales will increase in fiscal 2005 compared to 2004 though at a much lower growth rate than we experienced during the past year. Our forecast for fiscal 2005 primarily reflects our expectations that: 1 ; sales of Tri-Sprintec will increase year-over-year due to a full year contribution in fiscal 2005; 2 ; most of our products will continue to gain additional market share through increased generic substitution which will offset the negative effects of additional market competitors and declining prescriptions for some of these products; 3 ; in fiscal 2005, we will receive approval for and launch two new oral contraceptive products; and 4 ; we will add new customers during the year. Generic Products Other Sales of other generic products increased approximately 7% in fiscal 2004 as compared to the prior year period, primarily due to sales of our Mirtazapine Orally Disintegrating Tablet the generic equivalent of Akzo Nobel and Organon, Inc.'s Remeron SoltabTM ; , which we launched in December 2003, and sales of Claravis a generic equivalent of Roche Pharmaceutical's Accutanee ; , which we launched in May 2003. These increases were partially offset by a significant decline in sales of our Dextro salt combo product a generic equivalent of Shire Richwood, Inc.'s Adderall ; due to lower pricing and lower volumes resulting from the entry of two additional generic competitors. Proprietary Products Sales of our proprietary products more than doubled in fiscal 2004 as compared to the prior year. This increase relates primarily to: 1 ; sales from the four products we purchased from Wyeth in June 2003; 2 ; the launch of SEASONALE, our extended-cycle oral contraceptive; 3 ; increased sales of Cenestin; and 4 ; sales of Loestrin and Loestrin Fe, which we purchased from Galen Chemicals ; Limited "Galen" ; in March 2004. In September 2003 we received approval for our SEASONALE extended-cycle oral contraceptive. We began promoting SEASONALE directly to physicians in November 2003 and initiated our direct-to-consumer television and print advertising program during our fourth quarter. Since we launched SEASONALE, demand for the product as measured.

AHRQ has just released a booklet for older adults called The Pocket Guide to Staying Healthy at 50 + This guide incorporates new research-based recommendations from the U.S. Preventive Services Task Force. The guide was developed in partnership with AARP, and it updates the original Staying Healthy at 50 + published in 2000. The Pocket Guide, available in English and Spanish, includes tips and recommendations on good health habits, screening tests, and immunizations. It provides easy-to-use charts to help track personal health information and includes questions to ask health care providers, as well as resources to contact for additional information. The guide is available on the AHRQ Web site in English : ahrq.gov ppip 50plus and Spanish : ahrq.gov ppip 50plussp . Printed copies are available from the AHRQ Publications Clearinghouse by calling 800 ; 358-9295 or sending an e-mail to ahrqpubs ahrq.gov mailto: ahrqpubs ahrq.gov. Once or twice daily. Mild redness and scaling are common during the first few weeks. E. Azelaic acid Azelex 20% cream ; , a dicarboxylic acid with combined antimicrobial and comedolytic properties, is FDA-approved for mild-to-moderate inflammatory acne. It is massaged in twice daily. Mild skin irritation occurs in 5-10% of patients. Because azelaic acid does not cause photosensitivity, it is an alternative comedolytic agent for patients who are reluctant to refrain from activities that involve significant exposure to the sun. Hypopigmentation is a rare adverse reaction. F. Systemic agents 1. Oral antibiotics are the foundation of moderate-to severe inflammatory acne treatment because they reduce ductal concentrations of P acnes. Improvement can generally be seen within 2-3 weeks. 2. Tetracycline is favored because of its better tolerability and lower incidence of P acnes resistance is initiated at a dose of 1-2 g d in 2-4 divided doses. Tetracycline should be taken on an empty stomach. Many individuals whose acne is controlled can be weaned off oral antibiotics after 6 months of therapy, and then topical antimicrobial therapy can be continued for maintenance. 3. Long-term use is considered safe; the most common side effects are gastrointestinal upset and vulvovaginal candidiasis. Gram-negative folliculitis may occur, typically manifested by the sudden appearance of superficial pustular or cystic acne lesions around the nares and flaring out over the cheeks. 4. M i Minocin ; and trimethoprim sulfamethoxazole TMP SMX [Bactrim, Septra] ; have a place in treating some refractory cases. Minocycline can be particularly valuable for patients with treatment-resistant inflammatory acne. Minocycline, like all tetracyclines, is contraindicated in pregnant women and in children younger than 9 years of age because of potential adverse effects on developing bones and teeth. 5. TMP SMX is prescribed at a dose of 1 regular strength tablet, qd or bid. Hematologic and dermatologic side effects have restricted its use to patients with severe acne refractory to other antibiotics and to those who develop gram negative folliculitis secondary to long-term antibiotic therapy. G. Hormone therapy improves acne by suppressing sebum production. A triphasic oral contraceptive pill containing ethinyl estradiol, 35 : g, and norgestimate Ortho Tri-Cyclen ; has been shown to reduce inflammatory acne lesions by 40%. H. Oral isotretinoin 13-cis-retinoic acid [Accutane] ; is the only available agent with the potential to cure acne. Most patients are started at 0.5-1 mg kg qd or bid, typically for 15-20 weeks. Adverse reactions to isotretinoin include cheilitis, nose bleeds, dry skin and mucous membranes, and photosensitivity. Less common are arthralgias myalgias, headache, nyctalopia, and, in rare cases, pseudotumor cerebri. Isotretinoin can induce abnormalities in liver, hematologic, and lipid functions. Isotretinoin is a teratogen. Contraception must be ensured during use and for at least 1 month after. I. Comedone extraction is an office procedure used to disimpact obstructive acne lesions. The obstruct ing plug can usually be expressed after enlarging the pore with a 25-gauge needle. J. Intralesional corticosteroid injection can rapidly within 48-72 hours ; resolve large or recalcitrant inflammatory acne lesions and reduce the risk for scarring. A 30-gauge needle is used to inject 0.05 0.3 mL of a solution containing triamcinolone acetonide through the pore of the lesion. References: See page 195 and achromycin.
Teratos means "monster" or "marvel" in Greek. Modern teratology--the study of birth defects-- began in the 1940s.Shortly after World War II, scientists speculated that fallout from bombs could affect fetal growth. In the 1960s, science recognized a substance that had no harmful effect on the mother but damaged her baby: thalidomide. The identification of thalidomide as a teratogen led to examination of other agents that can cause deformities in the developing fetus. These can be infectious, chemical, environmental, physical, or maternal health factors. The birth defects they can cause are also varied, ranging from medically minor to life threatening. Cardiologists, endocrinologists, dermatologists, general practitioners, and other clinical specialists prescribe many medications to treat common ailments, including high blood pressure, thyroid problems, arthritis, and skin conditions. Inevitably, some of these effective medicines are also teratogenic. To help women using these medications avoid pregnancy--and the potential for birth defects--contraception counseling is an important responsibility for all health care providers. In this issue of Health & Sexuality, we examine how the reproductive health community can work with other medical specialists to help women who use teratogens prevent pregnancy. As partners in care, we can share knowledge and skills to ensure a wider contraception safety net. This is first in a series of publications and projects we have planned to bring health care professionals together in mutually beneficial, proactive ways, with the goal of improving the quality of care for patients. As an example of how such projects may evolve, we include information about Roche Laboratories' System to Manage Adcutane Related Teratogenicity S.M.A.R.T.TM ; program. Through S.M.A.R.T., dermatologists and reproductive health professionals work together to prevent pregnancy in women who use isotretinoin Accutanne ; , a teratogen widely prescribed for acne. Since this unique program began, although numbers of prescriptions for Accutanne have risen, pregnancy rates among women using the medication have fallen significantly. Reducing unintended pregnancy has a positive effect on public health and clinicians of all specialties can only benefit from the sharing of knowledge and skills.I would like to offer heartfelt thanks to this issue's special clinical advisor, Linda Dominguez, and to Roche Laboratories for the unrestricted grant that made this issue possible. 1. VETERINARY CODES As the NARMS project includes testing done on food and animal isolates, antimicrobials used in veterinary medicine are important. Currently, several animal-specific agents not included in the LOINC lists. I had a meeting last week with Jerry Sable, with the CDC NEDSS project, and he indicated that he believes that you have recently brought someone with a veterinary micro background on-board. That would be great new direction for further LOINC development. With respect to the NARMS project, the following animal-specific agents are not in the LOINC database: Avilamycin Flavomycin Salinomycin Tiamulin Tilmicosin Tylosin Virginiamycin. These claims might be made directly by consumers, healthcare providers or by pharmaceutical companies or others selling such products and adapalene.

Studies are under way to determine whether it is worthwhile for people who are exceptionally prone to skin cancer to take vitamin a supplements, accutanw or retin-a. The most recent program of a similar nature has been developed by the European Federation of Associations of Families of People with Mental Illness EUFAMI which has three courses under the name "Prospect." More information about Prospect appears on page 14. All these programs have been developed with generous foundation and pharmaceutical industry support. Anil Vartak of Pune, India, has also developed an approach, which we are pleas ed to review briefly on page 7 of this issue. Reaching More Families Despite the encouraging development of formal educational courses for carers, consumers and providers, given by self-help organizations, not all families can be reached. Reasons for this are often cu ltural, again because some view mental illnesses as shameful or fateful or mysterious, or simply because of the fear surrounding the mere words used for these illnesses. At a recent workshop titled "The Carer's Journey" held in Buckinghamshire, England, Professor Julian Leff spoke of some of the traditional concepts of mental illness in the developing world. Causes were ascribed to "angered an and advair.

Background: The functions of three zona glycoproteins, ZP1, ZP2 and ZP3 during the spermzona pellucida ZP ; interaction are now well established in mice. The expression of an additional zona glycoprotein, ZPB 4, in humans, led us to reconsider the classical mouse model of gamete interaction. We investigated the various functions of human ZP hZP ; during the interaction of spermatozoa with fertilised and unfertilised oocytes. Results: The hZP of fertilised oocytes retained their ability to bind sperm albeit less strongly than that from unfertilised oocytes ; , to induce an intraspermatic calcium influx through voltagedependent channels similar to that observed with hZP from unfertilised oocytes and to promote the acrosome reaction at a rate similar to that induced by the ZP of unfertilised oocytes 61.6 6.2% vs60.7 9.1% respectively ; . Conversely, the rate of hZP penetrated by sperm was much lower for fertilised than for unfertilised oocytes 19% vs 57% respectively, p 0.01 ; . We investigated the status of ZP2 in the oocytes used in the functional tests, and demonstrated that sperm binding and acrosome reaction induction, but not ZP penetration, occurred whether or not ZP2 was cleaved. Conclusion: The change in ZP function induced by fertilisation could be different in human and mouse species. Our results suggest a zona blocking to polyspermy based at the sperm penetration level in humans.
