PRIOR CRYOPRESERVATION OF EX VIVO EXPANDED CORD BLOOD CELLS IS NOT DETRIMENTAL TO 1 2 ENGRAFTMENT AS MEASURED IN THE NOD-SCID MOUSE MODEL. Julie A Wood , Chris G Milross , Cathryn M 2 1 Collins , Jamie Case , Robert E Nordon , Alison M Rice . Children's Cancer Institute Australia for Medical 2 3 Research, Radiation Oncology, Prince of Wales Hospital, Graduate School of Biomedical Engineering, University of New South Wales, Randwick NSW, Australia Small collections of cord blood CB ; make transplantation of adult patients difficult, if not impossible, and engraftment, even in small children, can be quite slow. Cytokine mediated expansion has been proposed to facilitate engraftment. As CBs are frequently frozen in one aliquot, patients are transplanted with unmanipulated cells, from which an aliquot has been reserved. This aliquot is expanded for 10 days and then infused into the patient. Data shows that engraftment is better when expanded cells are infused on day 0 rather than day 10 Shpall et al, McNeice et al ; . Additionally, the functional quality of the expanded product cannot be assessed pre-transplant, as the patient is treated and given the first dose of cells prior to expansion. Expansion and subsequent cryopreservation of CB upon collection would allow assessment of the quality of the expanded product pre-transplant, giving the clinician a more informed choice. We questioned whether cryopreservation of expanded cells would compromise their ability to engraft the NOD-SCID mouse. + CB CD34 cells were incubated for 7 days with SCF + FL + MGDF. Half of the expanded cells were cryopreserved prior to transplantation. Post-thaw nucleated cell recovery was 94%, whilst recovery of day 14 progenitors was only 17%. NOD + SCID mice were transplanted with either freshly expanded or expanded CD34 cells which had been frozen. Blood, bone marrow BM ; and spleen were monitored for human engraftment. Despite the loss of day 14 progenitors among the thawed expanded cells there was no difference in the rapidity of human engraftment. BM of mice transplanted with + thawed expanded CD34 cells showed significantly higher levels of human engraftment 13.903.82% ; than mice + transplanted with fresh expanded CD34 cells 1.680.93%, p 0.0064 ; . These results suggest that prior cryopreservation of expanded CB cells is not detrimental to the ability of the cells to engraft in NOD-SCID mice. 1. Never wake up a sleeping patient to give them narcotics 2. Never combine narcotics with another sedating drug: e.g. alcohol, sleeping pills, Benadryl, or anti-anxiety pills such as Valium and Xanax 3. Start with the lower dose that is prescribed, and take additional medication only if the pain is still not adequately controlled 30 45 minutes after taking the first dose. For example, if the prescription reads "10 30 cc every 4 6 hours as needed for pain", then start with 10 15 cc pain medicine on the first dose. If the pain is not adequately controlled in 30- 45 minutes, then add 10 15 cc more for a total dose of 20 30 cc. When the pain begins to return in 4 6 hours, you may want to start with the 20 30 cc total dose the next time 4. Every day or two, try to decrease the total amount of narcotic medication given, by: a. increasing the time between doses or b. decreasing the amount used each time or c. substituting plain acetaminophen for the narcotic Step 3: Ibuprofen Motrin, Wdvil ; is a good pain reliever and fever reducer. Unfortunately, some studies have shown an increased risk of bleeding after tonsillectomy after taking ibuprofen and similar drugs. If you have followed the guidelines listed above, and the patient still has uncontrolled pain, the patient may take ibuprofen, as long as you realize that the patient is increasing their chance of having a postoperative bleeding episode that may require a surgical procedure under a general anesthesia to control the bleeding. Step 4: Call our office if the patient's pain is not controlled using the above guidelines. Other Questions: For non-emergent questions, please call our office 503-581-1567 ; between 9: 00 and 3: 00 pm. Monday through Friday. For emergency questions, please call our office 503-581-1567 ; , and our answering service will page the doctor on call. We have a doctor on call 7 days a week.

Peralgesia, also permit certain drugs to treat the pain. This appears to be true for both acute experimental pain, such as that induced by capsaicin, and physiopathological pain, such as that seen after nerve injury. In states of chronic pain, feedback onto this descending modulatory circuitry from higher centers such as the amygdala and anterior cingulate cortex 20, 21 ; could further amplify spinal nociception, so that the perceived pain is greater. It may also be that, even in cases of relatively normal peripheral activity, abnormal central activity may lead to diffuse, widespread pain, as seen in fibromyalgia, migraine, and irritable bowel syndrome. Extensions of these studies in patients with chronic pain should shed light on the role of descending pathways in pathological pain states and reveal how such circuits impact upon treatment efficacy.

Two distinct GH-binding proteins GHBP ; have been identified in plasma of humans: 1 ; a 60 protein that is identical with the extracellular portion of the GH receptor and has high affinity for the 22 kD predominant ; form of GH; and 2 ; a low-affinity 100170 kD GHBP that may not be a single protein. The regulation and possible physiological significance of these proteins have been reviewed Mercado & Baumann, 1993 ; . Plasma concentrations of the high-affinity GHBP are quite stable throughout the day in a given individual. Current evidence suggests that GH does not regulate plasma GHBP levels to a significant degree. However, plasma GHBP concentrations are decreased in patients with malnutrition, insulin-dependent diabetes mellitus, hepatic cirrhosis, renal failure, hypothyroidism and critical illness. Oral oestrogen therapy and obesity are associated with increased GHBP levels. In normal subjects, GHBP concentrations are positively correlated with percentage body fat and measures of subcutaneous abdominal fat and intra-abdominal visceral fat Fisker et al., 1997 ; . Decreased or undetectable plasma levels of GHBP have been described in patients with certain types of congenital short stature Mercado & Baumann, 1993 ; . In normal children, plasma GHBP levels are inversely related to the 24-hour GH secretion rate Martha et al., 1991 ; . In GH-deficient children, the increase in IGF-I concentrations and growth velocity induced by treatment with GH is correlated with serum.
