W H Y Your doctor has prescribed MIRAPEX because it has been shown to improve the symptoms of Parkinson's disease. Sometimes MIRAPEX can be used alone--without other Parkinson's disease medications. In other cases it is prescribed in combination with such medications as carbidopa levodopa ie, Sinemet or Sinemet CR ; . Taken with or without another drug, MIRAPEX has been clinically proven to effectively reduce the symptoms of Parkinson's disease. H OW D MIRAPEX works by stimulating the dopamine receptors in your brain. Ordinarily, receptors activate by receiving dopamine, but, due to the damage caused by Parkinson's disease, your body does not naturally produce enough dopamine to fully stimulate these receptors. MIRAPEX acts as a substitute for the lost dopamine no longer produced by your brain. Bank clinical diagnostic criteria bradykinesia and at least one of muscular rigidity, 4-6 hz rest tremor, or postural instability not caused by primary visual, vestibular, cerebellar, or proprioceptive dysfunction ; .12 A convenience sample of 35 patients with `new-onset' parkinsonism not felt to be due to neuroleptic medication use or previously diagnosed idiopathic Parkinson's disease were studied further. To assess motor function, all sample patients completed 3 trials of the tap test `tap your hand as many times as possible in 15 seconds between 2 pie plates 20 cm apart' ; and 3 trials of Timed up and go TUG ; test `Stand, walk 3 metres, turn around, and come back, as quickly as is safely possible' ; . Patients then received 100 mg 25 mg of levodopa carbidopa and 10 mg domperidone to prevent levodopa-induced nausea and vomiting ; orally. Three trials each of the tap test and TUG test were repeated 1 hour after administration of levodopa carbidopa and domperidone. The mean of the 3 trials was taken at each stage; a positive levodopa response was defined as a 20% or greater improvement in either tap test or TUG test performance. Patients with a positive levodopa response were prescribed levodopa carbidopa 100 mg 25 mg twice daily with domperidone 10 mg twice daily, with dosage adjustment at the discretion of the attending physician. All patients with a positive levodopa response were reassessed for continued response at 1 week and 1 month after levodopa initiation. For all patients assessed, the diagnosis of dementia and dementia-type was made by the specialist in geriatric medicine providing ongoing care for the patient while in hospital on the geriatric inpatient unit. In most cases, results of a recent computerized tomography CT ; 61% ; or magnetic resonance image MRI ; 12% ; scan of the brain was available before discharge. All statistical analyses were performed with SPSS software version 9.0, SPSS, USA ; . The ability to predict `new-onset' parkinsonism was examined using multivariate analysis. RESULTS Of 1, 009 consecutive discharges, 51 were 65 years of age; discharge summaries were unavailable for 108 patients, leaving a sample of 850 patients. Of the patients, 42 5% ; were labelled as having parkinsonism by other physicians, but did not meet. Were costs and consequences valued credibly? Yes No Reviewers' comments Average costs rather than marginal costs were employed. It is possible that clinics for administration of these drugs would reduce the marginal cost. However, it is possible that these administrations should have been costed as a day case Only problem could be with the treatment of neutropenia. In the UK, this is unlikely to be treated and, therefore, even grades 34 would have zero resource use attached to this adverse event References for unit costs were clearly stated, consequences were from the clinical trial although some resource use data were from an expert panel.

Discussion: In this paper, we evaluated the different uptake pathways for pulmonary surfactant in a whole lung model. This experimental system enables the study of alveolar type II cells in their native environment with intact cell-cell interactions and avoids the unpredictable effects of enzymatic digestion needed for the isolation of type II cells as well as the phenotypic changes that occur rapidly in culture. In addition, by applying trace amounts of labeled liposomes to the natural and cutivate and carbidopa, for example, carbdiopa mechanism. Treatment modalities that have been shown to be effective for patients with ESRD and RLS include the intravenous administration of epoetin alfa 85 ; and the use of clonidine 218 ; and dopaminergic agents. 219-221 ; In a recent analysis by Janzen and colleagues of published studies on the use of levodopa in dialysis patients, the authors reported that carbidpa levodopa was safe and effective in managing RLS in this population, but cautioned that the dosage must be individually titrated and that rebound and augmentation should be monitored. The type of dialysis does not appear to affect RLS. 172.

Discussion There are no RCT studies in this area, but level 4 pre-post studies noted that the vacuum device is an acceptable alternative for ED therapy in men with SCI who may not tolerate other methods and whose hand function can warrant its use unless a partner applies it ; . Premature loss of rigidity, petechiae and penile skin edema were unwanted side effects. The maximum vacuum pressure should not exceed 250 mmHg to prevent petechiae and ecchymosis ; and the penile ring placed at the base of the penis to trap blood does not remain for more than 30 45 minutes. An alternative vacuum device Synergist ; is a vacuum device within a silicone sheath that remains on the penis that can be used for longer periods of time due to the absence of constricting bands and a much lower vacuum pressure 8 20 mmHg ; , and which the majority of patients found satisfying. Denil et al. 1996 ; reported on 20 couples where 93% of men with SCI and 83% of their female partners reported sufficient penile rigidity for intercourse obtained by the use of a vacuum device after 3 months, but by 6 months less than half the couples were satisfied with the device. Most side effects were temporary and minor. Conclusion There is level 4 evidence that supports the use of vacuum devices and penile rings as treatment for erection dysfunction in men with SCI.

Several times daily in phase with the "off" episodes and ultimately died of NMS 14 ; . There is also variability of onset of NMS among cases secondary to withdrawal of dopaminergic drugs but there are, to our knowledge, no reports that the syndrome developed more than a few weeks after discontinuation of anti-Parkinsonism medication. The syndrome usually evolves over a 72hour period 12 ; . When NMS is due to acute withdrawal of dopaminergic therapy, the first step in treatment is to reinstate therapy with these agents. Supportive therapy is essential and includes the use of cooling blankets and antipyretics for hyperthermia, correction of electrolyte abnormalities and fluid imbalances, blood pressure monitoring, nutritional support if the syndrome is protracted and hemodialysis for renal failure. Therapy with dantrolene is an established treatment of NMS of any cause Recently, carbidopa levodopa has been used effectively as an adjunct to or in place of dantrolene in management of NMS caused by neuroleptics 15, 16 ; . Finally, even though NMS is potentially a lifethreatening condition, not all cases of NMS result in serious consequences. A "benign" type of malignant syndrome consisting of moderate elevation of serial CK and worsening of motor symptoms in Parkinsonian patients with mild upper respiratory tract infections have been described 17 ; . CONCLUSION The timely diagnosis and appropriate treatment of NMS can be life-saving. Therefore, it is critical that the entire spectrum of causes of this syndrome be appreciated, including withdrawal, reduction, or alteration of dopaminergic therapy. In this regard, the term "neuroleptic malignant syndrome" must be considered inaccurate and misleading. "Acute dopamine depletion syndrome" has been suggested as a more appropriate term 1 ; , However, as it is likely that the original terminology for the syndrome will remain, it is perhaps simpler to always remember to refer to NMS, with qualification, as the neuroleptic not always ; malignant sometimes ; syndrome. ACKNOWLEDGEMENT The authors would like to thank Ms Shyamala Narayanaswamy of the Singapore General Hospital Drug Information Service for her prompt assistance which contributed to establishing the diagnosis in this case.
