RESULTS Effect of pH on clarithromycin and azithromycin zone diameter determined by disk diffusion method. NCCLS document M2-A5 11 ; permits a medium pH range of 7.2 to 7.4 when testing H. influenzae by the disk diffusion method in supplemented HTM agar. The zone diameters obtained with a 15- g clarithromycin disk were 1 to 3 larger on HTM agar at pH 7.4 than on HTM agar at pH 7.2 for 23 of 44 isolates and were 1 to 2 smaller at pH 7.4 than at pH 7.2 for eight isolates Table 1; Fig. 1A ; . By the disk diffusion method, the classification of all isolates was the same at both pHs; 42 isolates were classified as clarithromycin susceptible and 2 were classified as clarithromycin resistant. The zone diameters obtained with a 15- g azithromycin disk were also altered by the pH of the HTM agar. Zone diameters were 1 to 3 larger at pH 7.4 than at pH 7.2 for 20 isolates, and zone diameters were 1 mm smaller at pH 7.4 than at pH 7.2 for three isolates Table 1; Fig. 1B ; . All isolates were classified as azithromycin susceptible at both pHs. The quality control isolate H. influenzae ATCC 49247 had a zone diameter of 22 mm HTM agar at pH 7.4, which is out of the acceptable range of 13 to had an acceptable zone diameter on HTM agar at pH 7.2. However, 12 isolates were classified as clarithromycin susceptible at pH 7.4 and intermediate at pH 7.2; 1 of these isolates was the quality control isolate ATCC 49247. One isolate was classified as clarithromycin intermediate at pH 7.4 and susceptible at pH 7.2. Similarly, for 18 of 44 isolates azithromycin MICs were twofold higher at pH 7.2 than at pH 7.4 and for one isolate the MIC was twofold lower at pH 7.2 than at pH 7.4 Table 1; Fig. 2B ; . The MIC50 of azithromycin was 2 g ml both pH 7.2 and terbutaline.

The high serum and gastric juice urea concentrations in uraemic patients, the increased utilisation of urea by the urease of Helicobacter pylori can lead to an elevated concentration of gastric juice ammonia, a substance that has been incriminated in the development of chronic gastritis 2 ; . Studies on patients with normal renal function have shown that successful eradication of Helicobacter pylori led to healing of ulcers 6 ; , significant reduction of ulcer relapse rate 1, 7 ; , and a reduction of recurrence of intestinal type gastric cancer after endoscopic resection 8 ; . It expected that patients with impaired renal function could share these benefits. Various antimicrobial agents have been used in different combinations for eradication of Helicobacter pylori including amoxicillin, clarithromycin, tetracycline and metronidazole 9-10 ; . The optimal regimen in any particular region may be affected by the development of resistant strains to various antimicrobials, the incidence of side effects and compliance to the regimens 9-10 ; . A report from Niamh Arthur In September 2004, Malta fulfilled the requirements to become the 75th full member of the WHO International Drug Monitoring programme. Malta is a small island in the Mediterranean Sea between Sicily and Tunisia, with a population of some 400, 000 inhabitants, many of whom are bilingual, speaking both their native Maltese and English. As part of an EU Commission supported twinning project, the Irish Medicines Board IMB ; and Medicines and Healthcare products Regulatory Agency MHRA ; worked with the developing Maltese medicines agency, to establish their regulatory system, with the IMB responsible for training in the area of pharmacovigilance. EU-funded Twinning Projects aim to develop modern and efficient administrations in accession countries that can implement the Union's legislation to the same standards as existing Member States. The Twinning Project, between the IMB, MHRA and the Medicines Authority in Malta, provided the arrangement for the Maltese government to work with their Irish and British counterparts. The project, which began fourteen months before accession, in February 2003, finished recently in October 2004. Throughout this period, a member of the IMB staff, Ms Anne Gray worked at the Maltese medicines agency on a full-time basis, as Pre-Accession Adviser, co-ordinating all aspects of the twinning project. The wider objective of this project was to support the establishment of a competent regulatory authority in Malta, known as the Medicines Authority.
