En 1921, Wilder observ que una dieta alta en grasa y restringida en carbohidratos estimulaba los mecanismos adaptativos del ayuno, asociando este cambio metablico a disminucin de crisis, condicin que se mantena hasta que se instauraba la dieta normal. Se ha evidenciado que la presencia del cido betahidroxibutirato BOHB ; y acetoacetato AA ; , seran los responsable de la accin anticonvulsivante, debido a la que su accin a nivel cerebral aumenta la sntesis del cido aminobutrico GABA ; . Estudios han demostrado que la dieta es 100% efectiva entre el 7 al 33% de los nios con epilepsia refractaria y el 22% y 56% de ellos, las crisis disminuyen en 90%. No se ha encontrado correlacin entre la edad o el tipo de crisis y la disminucin del nmero de convulsiones. Pero si, que nios mayores de 8 aos controlan la crisis en 50%, y tienen menor tolerancia a la dieta. Las caractersticas de la dieta cetognica para producir cetosis son: proporcionar caloras entre 75-95 kcal kg da, aportar el 90% de lpidos y 10% protenas + carbohidratos, manteniendo una relacin 4: 1 o grasas: carbohidratos + protenas ; . Mantener el nivel en sangre de cido BOHB 4 mmol l se ha correlacionado a mejor control de la frecuencia de crisis. Otra de las grandes ventajas de la dieta es la disminucin o suspensin del tratamiento farmacolgico, asociado a la reduccin de crisis despus de dos semanas de dieta, lo que adems significa disminuir los costos del tratamiento en la epilepsia. Efectos adversos asociados son clculos renales, hipoglicemia hipercolesterolemia e hiperuricemia, y en mujeres adultas alteracin en ciclo menstrual o amenorrea. Considerando que la epilepsia en Chile tiene prevalencia de 32 casos cada 1.000 nios menores de 14 aos y la resistencia a terapia farmacolgica es en el 20%, especialmente en el tipo atnicas y mioclnicas 10% de la poblacin ; , la dieta cetognica debera ser considerada como tratamiento de primera eleccin en este tipo de pacientes. Estudios clnicos no controlados, realizados en el INTA, Universidad de Chile y Servicio de Neuropsiquiatra Infantil del Hospital San Borja Arriarn, en pacientes con epilepsia refractaria y Sndrome de Rett, han evidenciado resultados satisfactorios, logrndose un 30% de control de la frecuencia crisis epilpticas generalizadas, especialmente en el tipo atnicas y mioclnicas registrndose mejora evidente en el rea motora y social. Evidence. The evidence must be directed to and commensurate with the rejected claim or claims. Evidence directed to a general concept or idea will likely be disregarded. In addition to reviewing the 131 affidavit for evidence of prior inventorship of the claimed subject matter, the examiner is likely to evaluate any exhibits for possible use as evidence of prior publication use or sale or failure to disclose best mode, which are all activities that can form the basis to invalidate the claims.9 In any event, once evidence that will meet the substantive requirements for establishing prior inventorship is collected, it must be carefully reviewed. Relevant considerations include whether the evidence is likely to unduly narrow or define the invention. A judicious selection process ought to be part of the affidavit drafting process. As part of the selection process, careful consideration should be given to the effect the evidence may have after issuance of the patent.
