People increasingly use alternative remedies for chronic problems. It should be strongly noted that alternative or natural remedies are not regulated and their quality is not publicly controlled. [See Warning Box.] Non-Drug Therapies. Alternative non-drug therapies used for headache management include hypnosis, meditation, visualization and guided imagery, acupuncture, acupressure, yoga, and other relaxation exercises. There is no clear evidence that any of these techniques have specific value for migraines. Some studies report the following: Acupuncture. A number of individuals have tried acupuncture with varying response. In one 2003 study, it had some benefits in the early treatment of a migraine attack, but a triptan was more effective at relieving the headache. Relaxation Techniques. Muscle relaxation techniques may be helpful. One interesting 2001 study reported that relaxation treatments appeared to help adolescents with migraine but not tension headaches. Electrical Stimulation. Small studies have found that therapy with transcutaneous electrical stimulation TENS ; may reduce migraine headache episodes. The procedure involves a very mild electrical sensation across the skin. One course of TENS takes about a half hour. One report suggests that using TENS with acupuncture points, along with self-hypnosis and relaxation techniques, may be an effective management option among patients with migraine headaches, but long-term and well-conducted studies are needed to confirm this. Riboflavin Vitamin B2 ; . There is reasonable evidence on the benefits of vitamin B2 for migraine sufferers. In one study, patients who took 400 mg of vitamin B2 riboflavin ; reduced their migraine attacks by half, although the vitamin had no effect on the severity or duration of migraines that did occur. In another study, it helped increase the effectiveness of beta-blockers, drugs used to prevent migraines in some people. Vitamin B2 is generally safe, although some people taking high doses develop diarrhea. Feverfew. Feverfew is the most studied herbal remedy for headaches and is effective in some cases. It should be noted, however, that, like all effective headache remedies over use can cause a rebound effect. Some experts recommend purchasing feverfew in dried leaf form. It appears to be safe, but side effects can be distressing. Phytoestrogens. Phytoestrogens are plant products that have estrogen-like properties. They include soy and black cohosh. In one study, a combination of soy compounds, dong quai, and black cohosh was associated with a lower risk for menstrual migraines. Because of the association between oral contraceptives, which contain estrogen, and a higher risk for stroke in people with migraines, no one should take such remedies without consulting a physician. Glucosamine. Glucosamine is a natural substance being used with some success for patients with osteoarthritis. Anecdotal reports are suggesting that the use of this agent may prevent migraines. As with other natural remedies, products are not regulated by the FDA. It is being well studied for arthritis, however, and to date has few side effects. Many available brands e.g., CVS, Walgreens, Wal-Mart ; are manufactured with appropriate ingredients. Coenzyme Q10. Coenzyme Q10 is a natural substance that is important for the transport of electrons. One study reported that patients who took it experienced a significant reduction in migraine headaches after three months. Magnesium Supplements. Some studies have reported a higher rate of magnesium deficiencies in some migraine patients, such as those with menstrual migraines. Whether supplements are helpful, however, is unproven. On March 17, 1998, the parents of Brooks Brown, who had had an on-again, off-again friendship with Harris, reported to authorities that Harris had written on his personal Internet Web page that he would like to kill or injure people "like Brooks Brown." Brown's parents reported the threat to Jefferson County Deputy Mark Miller, who was one of the officers who had responded to the earlier van break-in incident see text accompanying notes 44-46 below ; . Miller filed a "suspicious incident" report from the Browns and took copies of the threatening entries. Sheriff's office personnel later said they could not access the Web pages, and so could not trace the sender; because the Browns wished to remain anonymous, the case was left open, and an application for a search warrant was never pursued to completion. After the massacre, the threatening Web pages were reviewed by staff members of the district attorney's office, who determined that " . based upon the information in the report to law enforcement, there would have been insufficient basis to legally support a request to obtain search or arrest warrants." Information about the threat also was forwarded to Jefferson County Deputy Neal Gardner, Columbine High's school resource officer. Gardner noted no untoward behavior by Klebold and Harris at school, and commented that the pair had "treated him with appropriate respect." A later taped television interview with Gardner established that Gardner did not know and thus could not identify Harris. At various times, the pair were accompanied by friends when they assembled and tested explosives from bomb-making recipes downloaded from the Internet. Klebold and Harris were arrested by Jefferson County Deputy Tim Walsh on January 30, 1998 after breaking into an electrician's van and stealing about $250 worth of items. Walsh came upon the pair in Harris' car going through the stolen articles. They were cited for criminal trespass, theft and criminal mischief. In written statements to police, Harris blamed Klebold for suggesting they break into the van, while Klebold said the pair had mutually decided to smash the passenger side window and steal the van's contents. Columbine has resulted in legislation in Alabama, California, Texas and Virginia that requires police to notify school authorities within 24 hours of the arrest of a student. They were assigned to the juvenile diversion program offered through the Jefferson County District Attorney's Office, which allows nonviolent, first-time offenders to receive deferred judgements and sentences if they make complete restitution to victims, undergo counseling and perform community service. Acute and chronic cellular stress activates a variety of signal transduction pathways that mediate the cellular response to injury. Such a response directs the cell to either repair and survival or cell death apoptosis ; . We have been using proteomics analysis to study in more detail the altered expression and in particular the phosphorylation status of proteins in the cellular stress response to toxicants. Protein tyrosine kinases are an important class of kinases that are crucial in cellular responses to the environment as well as in the development of cancer. These kinases mediate the phosphorylation of tyrosine residues in a variety of proteins thereby in ; directly affecting either gene expression or other cell biological events. The use of highly specific antibodies that recognize phosphorylated tyrosine residues in combination with proteomics technology allows the detailed identification of post-translational modified proteins in the cellular stress response. We have used this approach to better understand the molecular mechanism of toxicant-induced perturbations of cellular interaction cell-cell and cell-ECM ; in relation to cell death of, for example, piracetam sex. Ity for remediation of cognitive dysfunction.78, 79 One case study80 involving the assessment of clonidine a central -2 noradrenergic agonist ; found no benefit to this particular pharmacologic intervention. Nootropics are a relatively new class of CNSactive drugs that have a direct functional impact on the higher integrative mechanisms of the brain. A few of the nootropic-like drugs that have been advocated to improve cognitive function include piracetam, etiracetam, aniracetam, pramiracetam, vincamine, dihydro-ergotamine, and centrophenoxine. Their chemical structures are quite different and their specific mechanisms of action are still unknown. Some of the proposed mechanisms of nootropic action include facilitation of dopamine release, increase of acetylcholine turnover, and inhibition of -adrenoreceptors. One study81 in TBI did demonstrate some beneficial effects of pramiracetam. Unfortunately, most of the more promising nootropic agents are still unavailable for clinical use in this country. The beneficial effects of Hydergine, a dihydrogenated ergot alkaloid, were recently reviewed by McDonald, 82 and he concluded that it produced some global improvement in memory. However, a recent well controlled study83 using ergoloid mesylates for Alzheimer 's disease failed to show any significant memory benefit. No studies have specifically assessed the utility of this drug for treatment of cognitive dysfunction in individuals after TBI. Numerous drugs aside from Hydergine have been utilized to improve cognitive function secondary to their presumed beneficial effects on cerebrovascular blood flow. These drugs include papaverine hydrochloride, cyclanedelate, naftidrofuryl, and pentoxifylline. Although some literature suggests a beneficial effect of these agents in geriatric populations with concomitant "dementia, " there has been no substantial exploration of the benefits of these agents in persons with cognitive dysfunction following TBI. Given the neurochemical complexity of cognitive processes, it should not be surprising that pharmacologic agents may have the potential to actually impair cognitive processes in both noninjured and injured brains. It is critical to remain aware of the relative risks of certain pharmacologic agents in terms of their potential to impede cognitive processing. Three main classes of drugs are felt to have the potential to interfere with cognitive functioning by way of their basic neurochemical mechanisms of action: 1 ; catecholaminergic antagonists, 2 ; GABAergic agonists, and 3 ; cholinergic antagonists. Agents that block catecholaminergic receptor.

At the time of submission of the Proposal, the ECCB was to undertake only fiduciary responsibilities while the RCM was to have implemented the project. When project was transferred to the OECS Secretariat, as the PR, no one remembered to review and amend the Proposal. Consequently there was a delay of five months before project implementation went into full gear. This was further compounded by the delay in establishing HAPU which came into being in late June, although the Project had come into effect from March 1st. b. The Project was designed on the assumption that capacities at both the national and regional levels were sufficient to undertake a project of this size and complexity. The assumption was also made that the national health systems were sufficiently developed and resourced to undertake the management, care and treatment of a communicable disease of the nature of HIV AIDS. c. STUDIES ON DIFFERENT SOURCES OF SHANKHPUSHPI FOR MEMORY ENHANCING ACTIVITY Jai Malik, Maninder Karan, Munish Bhargava and Karan Vasisht University Institute of Pharmaceutical Sciences, Panjab University, Chandigarh-160014, India. Shankhpushpi, a well known medhya rasayana, used traditionally for its intellect promoting, anxiolytic, sedative and antiepileptic properties, is also helpful in curing leprosy, ulcers and urinary disorders. However, a review of literature has shown that different plants are used under the name shankhpushpi. In present investigations, different plants commonly used under the name shankhpushpi have been evaluated for memory enhancing property using widely accepted Elevated Plus Maze model. The activity of hydromethanolic extract of each plant was tested at four dose levels viz. 50, 100, 200 and 400 mg kg, against the standard drug piracettam 100 mg kg, p.o. ; and an amnesic drug scopolamine 3 mg kg, i.p. ; . The results showed that hydromethanolic extract of Convolvulus pluricaulis CPE ; 100 mg kg ; , Evolvulus alsinoides EAE ; 200 mg kg ; and Clitoria ternatea CTE ; 100 mg kg ; significantly reduced the transfer latency of mice by 70, 65 and 66 percent respectively with respect to their first day duration. The results were significant with respect to control animals. In comparison to scopolamine only CPE showed efficacy at all the four doses. The results were not significant at lower dose 50 mg kg ; of EAE and higher dose 400 mg kg ; of CTE. The above results showed that all the three plants possess memory enhancing activity with CPE showing strongest activity amongst the three.
